By using or accessing the health care and other professional clinical services ("Clinical Services") provided by Lavela Provider Network CA, P.C. or the non-clinical products and services ("Lavela Services") provided by Lavela Health Inc. (the Clinical Services and Lavela Services, collectively, the "Services") in any manner, you acknowledge that you accept the practices and policies outlined below, and you hereby consent that we will collect, use and disclose your information as described in this Privacy Policy.
Remember that your use of the Lavela Services is at all times subject to our Terms of Use and your use of the Clinical Services is at all times subject to the Patient Agreement, which both incorporate this Privacy Policy. Any terms we use in this Policy without defining them have the definitions given to them in the Terms of Use or Patient Agreement, as applicable.
As we continually work to improve our Services, we may need to change this Privacy Policy from time to time. We will alert you of material changes by placing a notice on the Lavela website, by sending you an email and/or by some other means. Please note that if you've opted not to receive legal notice emails from us (or you haven't provided us with your email address), those legal notices will still govern your use of the Services, and you are still responsible for reading and understanding them. If you use the Services after any changes to the Privacy Policy have been posted, that means you agree to all of the changes.
This Privacy Policy covers how we treat Personal Data that we gather when you access or use our Services. "Personal Data" means any information that identifies or relates to a particular individual and also includes information referred to as "personally identifiable information" or "personal information" under applicable data privacy laws, rules or regulations. This Privacy Policy does not cover the practices of companies we don't own or control or people we don't manage.

We retain Personal Data about you for as long as necessary to provide you with our Services or to perform our business or commercial purposes for collecting your Personal Data. Lavela Provider Network CA, P.C. is not a covered entity under the Health Insurance Portability and Accountability Act ("HIPAA"), nor is Lavela Health Inc. a business associate under HIPAA. Therefore, HIPAA does not apply to Lavela nor is the information we receive from you subject to HIPAA.
Medical information that you disclose to, or that is created by, Lavela Provider Network CA, P.C. and the physicians, allied health professionals, and other licensed professionals Lavela Provider Network CA, P.C. employs or engages as contractors is subject to the California Confidentiality of Medical Information Act ("CMIA") and will be used and disclosed by Lavela in accordance with CMIA. Medical information under CMIA is not subject to the California Consumer Privacy Act, as amended, and regulations issued under the California Consumer Privacy Act (collectively, the "CCPA"). Thus, your personal information is subject to and protected by the CCPA (as applicable) and may be used or disclosed in any manner permitted under this Privacy Policy, while your medical information is subject to and protected by CMIA, and may only be used and disclosed in accordance with CMIA.
